The plot will immerse users in the dark world of the Italian criminal underground of the 1900s. Players will have to master the rules of survival in a ruthless environment and go through a «tense criminal history». For the first time in the series, the action will take place in Italy. The game will be a prequel to the previous installments and will show the origins of organized crime in Sicily.

The developers have not yet made an official statement regarding localization. More details about the new part of the franchise are planned to be revealed in December 2024 during The Game Awards where the new trailer will most likely be shown.

The Mafia series began in 2002 with the release of the first installment, which told the story of the underworld of the American city of Lost Haven in the 1930s. The second part, released in 2010, moved the action to the Empire Bay of the 1940s and 1950s. Mafia III (2016) immersed players in New Bordeaux in 1968. In 2020, a remake of the original game called Mafia: Definitive Edition.

The Old Country is due out in 2025 on PC, as well as PlayStation 5 and Xbox Series X/S consoles.
